Are there ventilation holes on top?
No, there isn't any holes on top. The chin straps holds it in place well. Order this cap a size smaller than you usually wear. There is a lot of stretch.
No! But I added a small (1 inch) slit and it works great. Best way to be comfortable on those cool night dives!
No. Air does collect in it and I have thought about putting in a couple holes myself.

No ventilation holes on top. Just being a cap though trapped air doesn't seem to be as big an issue as it can be with a full hood.
I have not had a problem with air build-up in the beanie....ever! I do think there is a venting system but I am not near the beanie right now. I really like mine and have used it for over 75 dives.
No holes, I was able to make my own with a hot nail.

No, but I have not found that I need them. The beanie does not seem to be as prone as a regular hood to getting exhaust bubbles trapped inside.

No ventilation holes on top. It can get warm, but I also find this a plus when you want to skip a wetsuit.
