You hold the open end underwater with the inflated end protruding above.

After filling it with air....you just hold the open end bunched up in your hand...underwater a bit...and the whole thing sticks up quite high. SAVED MY LIFE ONCE when I was lost at sea in the Indian ocean! they SAW THIS LARGE ORANGE thing sticking up above the waves! I ALWAYS have it with me when I dive now!!!

You put your octo in the opening fill it and gather it and hold it. It's low tech but works great.

If held upright the air will stay in it. Otherwise a knot could be tied in the bottom, but this would take up a lot of length. It's definitely a cheap, no-frills (but perhaps more easily packable) safety sausage than the other offerings. If small and packable is what you're after, check out the small offering from XS. They're smaller than this, but pack smaller and are much more useable.

How wide is the opening at the bottom. I am thinking about using it as a sleeve for a Greenland style kayak paddle, for emergency signaling, and am wondering if it would fit (paddle blade width is approx 4")?

When flat, the whole tube is a tiny bit less than 6" wide. It does not narrow at the opening.
