JAMES S 22" is a medium , 23" could probably go either med or large. I would suggest trying a Medium first. The necks are a little long on these hoods.
CHRISTOPHER M I have a 22? head. I initially ordered a XL but later discovered that L was the correct size. The XL left to much room in the neck area. I have a 17" neck.
I also had an issue with the vent. The exhaust air from my regulator was entering around the opening and getting trapped in the top. I had to modify it to allow the air to flow freely.

I am 23" head too. I bought a M size. When I use it, I feel it's too tight for me. Now I think I?d like have tried a L size.
